MYSTERY has deepened over the death of a dad, his two daughters and sister-in-law after details of an A&E trip before the tragedy were revealed.
Bartlomiej Kuczynski, 45, his two daughters Jasmin, 12, Downreuang, seven, and their maternal aunt Kanticha Noon, 36, were all found dead on Friday.
The police watchdog has launched a probe into the quadruple tragedy in Costessey, Norfolk, over previous contact with the dad.
A missing person enquiry was made on December 14 after Bartlomiej vanished from the property.
He was later discovered close to his home and was taken to the Norfolk and Norwich University Hospital to undergo a mental health assessment.
But it has now emerged the dad left before any tests could be taken, with the hospital now releasing further details of his visit.
NNUH carried out a comprehensive review but the hospital does not provide mental health services, reports.
A spokesman said: "We can confirm that the patient was taken to the emergency department by police on December 14.
"Following a comprehensive clinical assessment, the patient was assessed as having mental capacity and he left the unit before being reviewed again by the team.”
It comes after The Independent Office of Police Conduct (IOPC) revealed details of a 999 call Bartlomiej made before the tragedy.

Police did not send any officers to the home and instead showed up an hour later when a dog walker raised concerns.
The watchdog said of Bartlomiej's call: "During that call the man expressed concerns for his own mental state saying he was confused.
"He was advised to seek medical advice and police did not attend.
"At around 7am the force received a call from a dog walker concerned for the welfare of those within the home.
"Officers attended shortly after and, tragically, found the man, a woman and two children dead."
The horror unfolded on Friday morning when police forced their way into the property.
Bartlomiej died from a single stab wound to the neck, while Kanticha suffered multiple knife injuries to her neck.
A post mortem on the two young girls is being carried out today.
Kanticha was visiting her sister's family home from her native Thailand when she was found dead.

The children's mum was at work as a caretaker when the tragic discovery was made, according to friends.
Norfolk Police previously revealed it is not looking for anyone else in connection to the deaths.
